Start here: the exercise checklist I actually use
Before I answer “how much,” I ask an owner to work through this list with me. It’s the same order I use on exam-room recheck days.
- Get the murmur graded and, ideally, imaged. A grade 1–2 murmur found on a routine exam is a very different conversation than a grade 5–6 murmur with a cough.
- Ask where the dog sits on the ACVIM staging system (A, B1, B2, C, or D) — this matters more than the murmur’s loudness for deciding what’s safe.
- Track resting respiratory rate at home, ideally counted while the dog is asleep, before you change any exercise routine.
- Watch for exercise intolerance — stopping mid-walk, excessive panting that doesn’t settle, or collapse-like wobbliness are reasons to stop and call the vet, not push through.
- Keep sessions short, frequent, and flat rather than one long, hilly outing, especially once a dog is in a heart-failure stage.
- Revisit the plan after every cardiology recheck, because stage and medication changes should change the exercise prescription too.
- Don’t ignore joint comfort — an older dog whose exercise is being trimmed back for heart reasons still needs a plan for the joints doing the work on shorter, more frequent walks.

Why the murmur’s grade isn’t the whole story
A murmur is a sound, not a diagnosis. It tells you blood flow across a valve is turbulent enough to hear, but it does not by itself tell you how much heart muscle or valve tissue is affected. The Merck Veterinary Manual notes that murmur intensity is a poor index of the severity of mitral regurgitation in dogs, with one caveat worth remembering: a soft, grade 1–2 murmur is almost always associated with mild disease, while a loud murmur doesn’t necessarily mean severe disease is present. This is where the ACVIM staging framework earns its keep. Dogs in Stage A (at risk, no murmur yet) and Stage B1 (murmur present, no meaningful heart enlargement) generally tolerate normal activity well. Stage B2 dogs, who have a murmur plus measurable heart remodeling, usually do fine with regular but not maximal exertion. Stage C and D dogs — those in or recovering from congestive heart failure — need exercise scaled well back and built around their symptoms on a given day, not a fixed mileage target.

A dog I still think about on recheck days
In my practice, I’ve watched this play out enough times that I have a mental file of it. A 9-year-old Cavalier King Charles Spaniel, about 17 pounds, came in because her owner noticed she’d started stopping halfway up the stairs to the second floor — something she’d never done before, and it had only been going on for about three weeks. Persistent coughing during or right after exercise, breathing that stays labored well past a normal cool-down period, gums that look pale or bluish, and any collapse or wobbly hind-end weakness are all reasons to stop and contact your veterinarian the same day. A single instance of any of these in a dog with known heart disease is worth a call, not a wait-and-see approach.
Owners sometimes ask whether it’s safer to just stop exercise altogether once a murmur is diagnosed. I’d push back on that. Deconditioning brings its own problems — weight gain, muscle loss, and joint stiffness — and none of those help a heart patient either. The goal isn’t zero activity; it’s activity matched to what the heart can currently support, adjusted as the disease stage changes.
Where joint and mobility support fits into the plan
Here’s the part that gets skipped in most heart-murmur articles, and it’s the part closest to my own lane. When a dog’s cardiac stage calls for shorter, more frequent, flatter walks instead of the long hikes they used to do, the biomechanical load on their joints doesn’t disappear — it just gets redistributed into more frequent stand-ups, more stair trips, more starts and stops. An older dog with early osteoarthritis who’s also managing a Stage B2 murmur is now asking those joints to work through more transitions per day, even on a lighter overall exercise budget.
I weigh what a supplement can realistically do for a dog’s diet, mobility, and recovery, and I want to be precise about what the evidence supports here rather than overselling it. Glucosamine and chondroitin remain among the most studied joint-support ingredients in dogs; in vitro and review work has looked at how glucosamine HCl may help limit experimentally induced cartilage degradation, per a review in the Journal of Veterinary Medical Science, while research summarized in Frontiers in Veterinary Science describes chondroitin’s role in supporting healthy synovial fluid homeostasis. The counterargument, and why I still land where I do
The strongest pushback I hear is from owners who’ve read that any exertion raises a compromised heart’s workload, so the safest move is to minimize activity as much as possible. That’s not baseless — exercise does increase cardiac output, and in advanced Stage C/D dogs, real restriction is appropriate. But applying that logic uniformly to every murmur, including a stable grade 2 found incidentally at a wellness visit, overcorrects. It trades a real but distant cardiac risk for a near-certain cost: deconditioning, weight gain, and joint stiffness that show up within weeks, not years. My position is to match activity to documented stage, not to the fact that a murmur exists at all, and to keep rechecking rather than setting the plan once and leaving it.
Where the near-term future of this decision gets easier
I expect echo-confirmed staging and simple at-home respiratory-rate tracking to keep becoming more standard parts of this conversation, which is a good thing — it moves the exercise decision away from “how loud is the murmur” and toward the actual data a cardiologist needs.
